Details:
Quite often spell icons on a hotbar get stuck in "queued action" mode (dark icons like recovering spell) but the spell can still be cast via this icon. They stay in "queued action" even after ur target is already dead. The issue gets fixed upon relogin

Just noticing this for the first time myself. With nothing targeted, I almost expect spell gems to be darkened like I have no target, so they are dulled out (though I don't think that's how Live works).

But after my fight example posted elsewhere, I have 5 icons, 1 is lit up, the others are dark - and one is Provoke (dark) and Provoke II (light) with nothing targeted. All my other level 2 Guardian arts are dark. In the knowledge book as well.

This is latest SVN + 6196L client.
