SpawnRemove / ProcessConditional collision

a lot of the spawn_list iterator usage check if itr->second is NULL, but ProcessSpawnConditional did not. Not sure there is much value keeping dead spawn ids on the iterator as a NULL entry. But this prevents the crash and the need to check for null on the itr->second..
